What do I need to do prior to my appointment?


​All new clients must fill out a new client intake form. Anyone seeking assistance with diagnosed illnesses must bring a referral from their physician prior to their appointment. You can retrieve your referral form here . Please take note that Medical Nutrition Therapy assessments will be monitored under the direct supervision of a licensed nutrition/dietitian and all information discussed about nutrition information in direct relation to disease will be monitored by the supervisor at all times. This basic health coaching session, which is not a MNT visit, is geared to help set goals, review and advise on meal planning and preparation, and review current lifestyle for advice. With the basic health coaching visit you do not need a referal from your physician becuase we will not be discussing medical nutrition and you will not be given disease-specific recommendations.




Where can I find the initial intake form?


The initial intake form, as well as all disclaimers, food journal, and consent forms will be automaticaally generated and given to you to fill out when you sign up and schedule your visit.




How do I acquire a referral from my physician?


The referral form required can be found on the forms page or you can retrieve your referral form here.




How much will this cost me?


The Nutrition Blog Membeership is $11. Basic Health Coaching visits are $50. Miliatry and Low-Income indivdials recieve a 25% discount on basic health coaching visits. The initial medical nutrition therapy (MNT), Nutrition Data Report will be $295.00. Each follow-up appointment beyond the two sessions will fall in a different price range. The initial medical nutrition therapy (MNT), CAGED Protocol will be $595.00. Each follow-up appointment beyond the 5 sessions will fall in a different price range. Follow-ups beyond your "already purchased" sessions are $37.50.




Where can I find more information about other clients that have had you as a nutritionist?


Unfortunately, there are federal laws against practitioners, coaches, and businesses giving out personal information about clients. Therefore, we would not be able to furnish that information. However, clients are allowed to leave reviews at their own will detailing their experiences as customers and consumers. You can visit Google, Facebook, or the website to see local reviews.




What does the Basic Health Coaching visit cover?


The basic health coaching session is geared to help set goals, review and advise on meal planning and preparation, and review current lifestyle for advice. The basic health coaching session is not a Medical Nutrition Therapy visit and is solely Basic Health Coaching. MNT assessments are a different type of evaluation that discuss nutrition in relation to disease status. These are monitored under the direct supervision of a licensed nutrition/dietitian. Meaning in your Basic Health Coaching visit, we cannot discuss nutrition information in direct relation to disease. Please refer to the footer of website to retrieve license or certification information, or visit the License Info link under the About tab.

Are you a physician?


No, I am not a physician. I am a clinical nutritionist; and as a Nutrition Educator and Health Coach, I am NOT A PHYSICIAN. This means:
-I do not diagnose disease. What I do is assist in providing nutrition education or Medical Nutrition Therapy depending on the severity of the case. This is not to be confused with conventional medicine or conventional medicinal techniques. -I do not prescribe medications or follow the basic stepping ladder of a medical physician. I use a holistic approach to gain wellness and optimal health by looking at the whole body and trying to find the “root cause” of your problem. Nutrition Education:
-Reinforcement of basic or essential nutrition-related knowledge. Medical Nutrition Therapy:
-Nutritional diagnostic, therapy, and counseling services for the purpose of disease management, which are furnished by a registered dietitian or licensed nutrition professional. Licensing:
-Licensed dietitians and nutritionists hold one of these credentials RD (registered dietitian), LDN (licensed dietitian/ nutritionist), CNS certified nutrition specialist). There are other credentials but the government only licenses those with one of these to practice legally. Medical Nutrition Therapy (MNT) is a specific application of the Nutrition Care Process in clinical settings that is focused on the management of diseases. MNT involves in-depth individualized nutrition assessment and duration and frequency of care using the Nutrition Care Process to manage the disease. Jasmine Blake Hollywood, assists clients in their desire to support the innate healing response of their bodies by suggesting an individualized selection of foods, herbs, nutritional supplements, relaxation, visualization, and exercise programs to help reduce inflammation, weight accumulation, and imbalances in biochemistry that may be the cause of disease states, conditions, or disorders.




Are you part of any associations or professional organizations?


For your knowledge, Jasmine is a member of the American Nutrition Association, American College of Nutrition, American Nutrition Society, American Association of Drugless Practitioners, American Psychological Association, American Thyroid Association, and other Military organizations such as the American Legion. Jasmine is not endorsed by these organizations.





Policies

Appointment Time-Frames


APPOINTMENTS AND CANCELLATIONS
The standard meeting time for initial consults is 1-2 hours and follow-ups are 30 minutes. It is up to you, however, to determine the length of time of your sessions, and you may purchase more than 1 session. Health Coaching initial consults are 30 minutes and follow-ups are 30 minutes. Even if a session is less than 30 minutes it will be logged as 30 minutes because 30 minutes is the amount of time blocked off on the calendar.




Service Fees


We don't accept checks! A 10.00 service charge will be charged to accounts that use credit cards or debit cards that decline at the time of charging. Thus, the amount you owe, plus $10.00 will be your new outstanding total. A 25.00 service charge will be charged to accounts that have recurring payments and payment has been claimed as faulty payment. Thus, the amount you owe, plus $25.00 will be your new outstanding total. A 3% service charge is charged to all electronic transactions. 7% Tax is included in all pricing, accroding to state and federal business law requirements.




Cancellations


Please remember to cancel or reschedule 48 hours in advance. You will be responsible for the entire fee if cancellation is less than 48 hours. This means: booked appointements that are no-call/ no-show will be subject to full charge if a cancellation notice is NOT received at LEAST 48 HOURS in advance. For this rule to NOT apply, there must a viable reason for exclusion. This is necessary because a time commitment is made to you and is held exclusively for you. If you are late for a session, please note you may lose some of that session time. The re-scheduled sessions that are made in response to a cancellation will be charged as normal visits, and payments previously made will be honored. TELEPHONE ACCESSIBILITY
If you need to contact me between sessions, please leave a message on my voicemail. I am often NOT immediately available. However, I will attempt to return your call within 24 hours. A text can be reasonable if it is an emergency. Please note that face-to-face sessions are highly preferable to phone sessions. Therefore, video links will always be sent. However, in the event that you are out of town, sick, or need additional support, phone sessions are available. If a true emergency situation arises, please call 911 or any local emergency room, and please leave a message on my machine regarding your current situation.




Social Media and Telecommunication


Due to the importance of your confidentiality and the importance of minimizing dual relationships, I do not accept a friend or contact requests from current or former clients on any social networking site (Facebook, LinkedIn, etc). If you are a current friend, your status as a friend will be removed for professional reasons. All clients, however, are able to join the private Facebook group Beauty Is Nutrition and maintain a neutral professional status on the Facebook Jasmine Blake Hollywood business page. Your status as a client will not be released to any persons within the Facebook community group or business page and we ask that you reframe from identifying yourself as a client or the liability clause is in the void. Groups are not for diagnosing but can be sharing information. Any misconduct or illegal acts, will result in removal from the group and dismissal from the client-practitioner relationship. No friends are to be added to this group. Beauty Is Nutrition is a private group and is for direct clinical relationships only. I believe that adding clients as friends or contacts on these sites can compromise your confidentiality and our respective privacy. It may also blur the boundaries of our therapeutic relationship. If you have questions about this, please bring them up when we meet and we can talk more about it.




Refund Policy


What if I schedule an appointment and can't make my appointment, do I get a refund? Unfortunately, Discover Your Greatest Self has a no refund policy in effect. Therefore, there are no refunds. So, please ensure you schedule a time and day that you will be able to make. What if I cancel before my appointment day, will I be eligible for a refund? Cancellations require a 48-hour verbal or written notice. Again, Discover Your Greatest Self has a no refund policy in place. Therefore, no refunds will be reimbursed even with a 48-hour cancelation notice. It is suggested that if you cannot make your appointment, even with an emergency, that you please contact me prior to your scheduled appointment to reschedule your appointment for a later date.
