Charge The initial cost for intake is $300.00. Clients who qualify for a sliding scale receive a reduced fee of $70.00. Please register here. To qualify for complimentary services, clients are required to meet specific eligibility criteria. Clinical treatment is based on funding availability and only accommodates a limited number of clients. All paperwork is required at the time of booking or full service charge will be applied. New Client Consultation Description Anyone who has received no professional services from the physician, or has a referral from a physician, may apply to become a new client. Nutrition Focused Physical Exam If you are meeting in person, a nutrition focused physical exam will be conducted to identify risk factors for chronic diseases. A different method can be conducted for online visits. Clinical Visits All clinical assessments will include a full evaluation of intake form, food diary, and submitted labs, food sensitivity tests, or nutrigenetic testing. A customized plan consisting of nutritional, herbal, and lifestyle recommendations will be provided that all requirements are in compliance. Follow Consultation Description This type of appointment is for the routine monitoring of any specific conditions or problems you may have, the status of your intervention, discuss more findings, or change your current protocol. These appointments are currently being offered via telehealth only. Location In-person when requested for scheduling in advance. Rates may vary. Virtual visits using secure Zoom platform, scheduled accordingly.
